What we do
Our substance misuse service offers structured support to people on release from police custody, prison or court to community. The focus is on resettlement and support for change.
The services provides:
- Three to six months of support including comprehensive assessment, care planning, and an intensive support package to empower people to take control of their recovery.
- Access to prescribing, and referral to wraparound services before being transferred to community substance misuse support within the East Riding Partnership Community if needed.
- Harm reduction – reducing the risk of harm, relapse prevention and overdose information – helping to tackle addiction by supporting to identify risky situations, triggers and responses.
- Help to identify unhelpful beliefs, values and thinking patterns, challenging these to be able to modify actions and identify alternatives to improve outcomes in line with their personal goals.
We support service users in becoming and/or remaining drug and alcohol free with a personal recovery plan, and excellent aftercare support. Whether prescribed opiate substitute medication or not, people who use our service can access support if they’ve been a drug or alcohol user and become abstinent while in prison.
We have links to specialist drug and alcohol services to provide community detoxification, access to inpatient detox or long-term day or residential rehabilitation. We can also provide links to self-help support such as SMART, AA and NA.
