Intensive Health & Resettlement Support – Wandsworth

What we do

Nacro’s Intensive Health Resettlement service supports people with their resettlement needs from the day of release. This includes working with external partners such as housing providers and mental health teams. We also support people with welfare benefits, registering with a GP practice, and opening bank accounts.

We assess a person’s needs before they are released from custody and work with agencies to ensure both a smooth transition to their community and a joined-up approach between healthcare and statutory resettlement.

People can be met at the gate upon release and escorted to all of their crucial appointments on the day, such as probation and local authority housing. We also make sure they have all of the essentials such as their correct medication, prescriptions and planned appointments for the first few days.

Support continues for 6-12 weeks to ensure the person gets appropriate safe and secure accommodation, access to welfare benefits, re-engages with health services and strengthen links with family and community support services.

Eligibility

To be eligible a person must have been sentenced and being released into a London borough. They must have had aΒ clinical diagnosis of a mental health conditions and/or be on certain types of prescribed medication.

How to refer

Referrals are submitted as a task via NHS System1 by staff working in prison healthcare.

We request that referrals are made three months before the service user’s release.

I have been helped twice now when I have been released from prison by Nacro. My support worker has met me at the gate and helped me attend my appointments. She has also kept communication with probation and other people when I am having bad days. I am so happy she is here to support me and I honestly do not know where I would be without Nacro, even when I mess up she stays patient with me. I cannot read or write so she assists me with filling in forms and reading my letters. She also checks up on me most days, it’s good to know someone has you in their best interests and wants to help you out.
